Doğal doğumda bebeğin başı vajina kanalından geçerken kaslar ve bağ dokusu aşırı gerilir. Bu durum özellikle iri bebek, uzun süren doğum veya müdahalelerde daha belirgindir. Sonuçta vajina duvarlarında elastikiyet kaybı ve pelvik taban kaslarında zayıflama meydana gelir. Bu değişiklikler zamanla kısmen düzelse de çoğu kadında kalıcı etki bırakır.
